Algebra, chapter VII, § 5. Endomorphisms of vector spaces

— Let UU and VV be two square matrices of order nn with coefficients in a commutative field KK. If there exists an invertible square matrix PP over some extension K′K' of KK such that V=P−1UPV = P^{-1}UP, then there exists an invertible square matrix QQ over KK such that V=Q−1UQV = Q^{-1}UQ.

Let EE be a finite dimensional vector space over a commutative field KK, let (ei)1≤i≤n(e_i)_{1 \leq i \leq n} be a basis of EE and let uu be an endomorphism of EE. Then by the exact sequence (1) of VII, p. 29, the K[X]K[X]-module EE, associated to uu is isomorphic to the quotient of the free K[X]K[X]-module E[X]E[X], with basis (1⊗ei)(1 \otimes e_i), by the image of E[X]E[X] under the K[X]K[X]-linear map X−uˉX - \bar{u}. The similarity invariants qi(X)q_i(X) of uu (VII, p. 32, Def. 2) are thus the invariant factors of X−uˉX - \bar{u} (VII, p. 22). Thus Prop. 6 of VII, p. 22, implies:

Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
