Algebra, chapter IX, § 1. Formes sesquilinéaires

Given a bilinear mapping Φ\Phi from E×FE \times F into GG, the mapping dΦd_\Phi from FF into LA(E,G)\mathscr{L}_A(E, G) (resp. the mapping sΦs_\Phi from EE into LB(F,G)\mathscr{L}_B(F, G)) characterized by (5) is called the linear mapping associated on the right (resp. on the left) with Φ\Phi.

Conversely, the data of a B-linear mapping dd from FF into LA(E,G)\mathscr{L}_A(E, G) (resp. of an A-linear mapping ss from EE into LB(F,G)\mathscr{L}_B(F, G)) determines uniquely, by the formula

Φ(x,y)=d(y)(x)(resp. Φ(x,y)=s(x)(y)) \Phi(x, y) = d(y)(x) \quad \text{(resp. } \Phi(x, y) = s(x)(y))

a bilinear mapping Φ\Phi from E×FE \times F into GG, of which dd (resp. ss) is the linear mapping associated on the right (resp. on the left).

Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
