Algebra, chapter IX, § 1. Formes sesquilinéaires

Soient KK a commutative field of characteristic 2, AA a separable quadratic extension of KK; one has A=K(θ)A = K(\theta), where θ\theta is root of an irreducible polynomial X2+X+βX^2 + X + \beta of K[X]K[X] and the KK-automorphism JJ of AA, distinct from the identity, is such that θJ=θ+1\theta^J = \theta + 1 (Chapter V, § 11, Exercise 8). Now prove that if EE and GG are vector spaces over AA, Φ\Phi a sesquilinear mapping (for JJ) from E×EE \times E into GG, one has, putting Q(x)=Φ(x,x)Q(x) = \Phi(x, x),

Φ(x,y)=Q(θx+y)−βQ(x)−Q(y)−(θ+1)(Q(x+y)−Q(x)−Q(y)). \Phi(x, y) = Q(\theta x + y) - \beta Q(x) - Q(y) - (\theta + 1)(Q(x + y) - Q(x) - Q(y)).

Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
