Algebra, chapter IX, § 1. Formes sesquilinéaires

Let E,E1E, E_1 be two AA-modules, Φ\Phi (resp. Φ1\Phi_1) a sesquilinear form on EE (resp. E1E_1). Suppose that Φ1\Phi_1 is non-degenerate and that there exist an element α∈A\alpha \in A and a bijection uu of EE onto E1E_1 such that Φ1(u(x),u(y))=Φ(x,y)α\Phi_1(u(x), u(y)) = \Phi(x, y)\alpha for all x,yx, y in EE. Show that: 1° Φ\Phi is non-degenerate ; 2° uu is linear ; 3° if E1E_1 is a faithful AA-module, the same is true of EE, and α\alpha is not a right divisor of 0 in AA ; 4° if Φ1\Phi_1 takes values in AA which are not left divisors of 0, the same is true of Φ\Phi.

Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
