Topologie algébrique, chapter IV, § 3. Groupes de Poincaré des groupes topologiques

Two locally connected and simply connected topological groups which are locally isomorphic are isomorphic.

Let G and G′G' be locally connected topological groups, let V and V′V' be connected neighbourhoods of the identity element of G and of G′G' respectively, and let f:V→V′f: V\rightarrow V' be a homeomorphism which is a local isomorphism of G onto G′G'. By corollary 1, there exists a unique continuous group homomorphism φ:G→G′\varphi : G\rightarrow G' extending ff and a unique continuous group homomorphism φ′:G′→\varphi ': G'\rightarrow G extending f−1f^{-1}. The homomorphisms φ′∘φ\varphi '\circ \varphi and IdG_G coincide on a neighbourhood of ee in G, hence are equal, since G is connected. Analogously, φ∘φ′=\varphi \circ \varphi '= IdG′_{G'}, whence the corollary.

Bourbaki sets the proof directly after the statement, and the corpus does not separate them, so what is above is the statement and its proof together.
