Commutative Algebra, chapter VII, § 4. Modules over integrally closed Noetherian domains

Let AA be a Dedekind domain and TT a finitely generated torsion AA-module. There exist two finite families (ni)i∈I(n_i)_{i \in I} and (pi)i∈I(\mathfrak{p}_i)_{i \in I}, where the nin_i are integers ≥1\geq 1 and the pi\mathfrak{p}_i elements of PP, such that TT is isomorphic to the direct sum ⨁i∈I(A/pini)\bigoplus_{i \in I} (A/\mathfrak{p}_i^{n_i}). Further, the families (ni)i∈I(n_i)_{i \in I} and (pi)i∈I(\mathfrak{p}_i)_{i \in I} are unique to within a bijection of the indexing set.

This follows from no. 4, Theorem 5, taking account of the fact that a pseudo-isomorphism is here an isomorphism.

Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
