General Topology, chapter VII, § 2. Continuous homomorphisms of $\mathbf{R}^n$ and its quotient groups
Every continuous homomorphism of into is of the form , where is the canonical homomorphism of onto (identified with ) and is a linear mapping of into .
Let be a continuous homomorphism of into . We shall show that there is a linear mapping of into such that the homomorphisms and coincide at all points of a neighbourhood of in ; the proposition will then follow from Corollary 1 to Proposition 2 of no. 2. Now let be a neighbourhood of in such that , restricted to , is a local isomorphism of with , and let be the inverse of , defined on . Since is continuous, is a neighbourhood of in ; the mapping
restricted to , is a continuous mapping of into such that for each pair of points in such that and ; hence (no. 2, Corollary 1 to Proposition 2) this mapping coincides with a well-determined continuous homomorphism of into at all points of a neighbourhood of o in . By Proposition 1 (no. 1) is a linear mapping of into ; for all we have therefore , which completes the proof.
Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
