Topological Vector Spaces, chapter IV, § 1. Duality

Let uu be a continuous linear mapping from E1E_1 into E2E_2.

(i) If E1E_1 and E2E_2 are Hausdorff then uu is injective if and only if the image of tu^t u is dense in E1′E'_1 for the weak topology σ(E1′:E1)\sigma(E'_1 : E_1).

(ii) For tu^t u to be injective it is necessary and sufficient that the image of uu is dense in E2E_2.

A vector subspace of E2E_2 is dense for the initial topology if and only if it is dense for the weakened topology (IV, p. 4, prop. 2). Prop. 5 then follows from II, p. 47, cor. 2.

Bourbaki puts the proof directly after the statement, so what is above is the statement and its proof together.
